HOME Choice

The Ohio Department of Medicaid initiated the HOME Choice program in October 2008. As of July 2016 over 8,253 people have transitioned from long-term facilities to home and community-based settings. HOME Choice is available to individuals of any age.

To be eligible for HOME Choice, participants must:

  • Live in a nursing facility, intermediate care facility for developmental disabilities, Residential Treatment Facilities or hospital for at least 90 days at the time of discharge;
  • Have an institutional level of care;
  • Be eligible for Medicaid at the time of discharge;
  • Have at least one Medicaid claim while living in the institution;
  • Relocate to a qualified residence.

 

Recovery Requires a Community

Recovery Requires a Community helps individuals diagnosed with mental health or substance use disorders by providing financial assistance for transitioning from nursing facilities to sustainable community living. This Ohio Department of Mental Health and Addiction Services program provides resources to individuals by supporting person-driven transitions to integrated community-based settings with supportive services.

Funds can be used in many ways to help individuals transition to the community when no other funding sources are available. Funding requests are approved on a case-by-case basis. Program Information is available on the OhioMHAS website at https://mha.ohio.gov/RRaC.
